I'm a little confused by this.. Is this a question? (there's no question mark..) Also I assume you mean consistently wrong? I've been to Taco Bell multiple times lately over the past several months and maybe once got something wrong in my order.

Have they ever got it right in the first place?

Taco Bell usually gets my order right. Sometimes they don't, but that's not very often.

Speaking of this, the last time I went to Taco Bell they gave me someone
else's entire order by mistake. In that order were wings! I had no idea Taco Bell even had wings.. but it was a pretty cool mixup because they weren't actually that bad. I would have never known otherwise! :)

Having been in the backrooms of a number of fast food places that have sketchy standards, I'm thinking it could be Sadness to eat at one in teh first place.

Seen enough that if I need quick-food I go to places where their kitchen can bee seen aby the customers, unless I have seen their kitchen myself.

There are particular outlets that hold themselves to very high standards and some that are roach hotels in most any brand, and I really prefer to see some of the steps in preparation myself.

How one prepares and stores the food also affects public health of the customer. You can easily send a person to the hospital.

My son was impressed by going through the In-and-Out drive through and
seeing them slicing whole potatoes and popping them into the fryer instead
of dumping a 20 pound sack of frozen french fries into the fryer.

The In-N-Out in Keizer, Oregon is still very busy. It's been there for about 2 and a half years, and there are still long lines at the drive-through. I visited the place once, this past July; I went inside and waited about 40 minutes for my food. It was good, but I don't plan on going back there any time soon. The wait is just too long.
